Concrete pour request template
A concrete pour request is what the contractor sends the consultant before a pour, so the formwork, the rebar and the embedded items are inspected and the pour time is approved. The file is free, in Excel, with no sign-up, on one sheet: the request details, the volume worked out by formula, the pre-pour checklist, the consultant's decision and what is recorded after the pour.

Why write a pour request?
- Because concrete cannot be undone: what is not inspected before the pour can only be checked afterwards by breaking it.
- So the request names the element, its grid lines, its level and the drawing it was built to, and the inspector knows what to check.
- So the pour time is booked with the batching plant, the pump and the consultant at once.
- And so the signed request proves the element was inspected and approved before the pour, for the quantity survey or any dispute later.
What is in the file?
- One right-to-left sheet set to print on A4 portrait.
- The request details: number, date, building, element, grid lines, level, drawing, concrete grade, pour method, concrete source and planned time.
- A «بنود الصب» (pour lines) table that works out each line's volume from count, length, width and depth, then the total, the waste allowance and the volume to order from the plant.
- A seven-item pre-pour checklist, each item with its result, note, inspector and signature, then the consultant's decision and notes, the signatures and what is recorded after the pour.
- One example request filled in and marked «مثال», and empty lines with their formulas.
How to fill it, step by step
- Give the request a new sequential number, then type the building, element, grid lines, level and drawing reference.
- Type the concrete grade as in the drawings, the pour method, the concrete source and the planned time.
- In «بنود الصب» (pour lines) type the count and dimensions of each part, and the quantity and total are worked out. Type the waste allowance you use and the volume to order from the plant appears.
- Walk the seven items with the inspector of each discipline, type the result and note, and each inspector signs next to their item.
- Fix what has notes and have it re-inspected, then send the request to the consultant for a decision: approved or rejected.
- After the pour, record the start and end times, the actual volume and the number of samples, and compare the actual volume with the calculated one.

An example from the file: the second-floor roof
A pour request for a slab with downstand beams, with the example's dimensions:

The beam depth in the example is the part below the slab only, because the part inside the slab thickness is counted with the slab. The waste allowance is for illustration: type the one your company uses.
Practical notes
- Send the request early enough for the inspection and any fixes, within the period your contract or your consultant agreement sets.
- Do not call the plant before the request is approved: a truck waiting on site is cost with no work.
- If a slab is poured in several parts, write one request per pour and mark the approved construction joint.
- Keep the delivery ticket numbers with the request, to match the actual volume with the calculated one.
